2. Information We Collect
We collect: your name, contact details (phone, email), health information (medical history, treatment records, ACC claim details) through our booking system and consultation forms. We also collect anonymised website usage data via Google Analytics 4.
3. Why We Collect Information
We collect personal and health information to: provide safe and effective treatment, manage ACC claims, communicate about appointments, comply with our legal and professional obligations under the HPCA Act 2003, and improve our services.
4. Health Information Privacy
As a healthcare provider, we are bound by the Health Information Privacy Code 2020 (HIPC). Your health information receives additional protections beyond the standard Privacy Act 2020. We will only share health information with other healthcare providers with your explicit consent, or as required by law.
5. Who We Share Information With
We may share information with: ACC (for injury claims), your GP or other healthcare providers (with your consent), our booking system provider (BookWell), and Google Analytics (anonymised data only). Your data may be stored on servers located outside New Zealand.
6. Data Security & Retention
We take reasonable steps to protect your personal and health information from unauthorised access, use, or disclosure. Health records are retained for a minimum of 10 years as required by the Health (Retention of Health Information) Regulations 1996.
7. Your Rights
